Clinton Veterinary Clinic takes your privacy seriously. Your pet(s)’ records “may not be released except by court order, a public health emergency, consent of the client, or otherwise authorized by law” (Iowa Administrative Code - Rule 811-12.4). We are required by law to report any knowledge of an animal bite involving a person – even if the bite occurs during the visit at the clinic. We are also required to report any exotic or foreign animal disease. You will be notified when we are required to file a report. We report any Rabies vaccines given to Illinois dogs to the county of residence and cats only to Whiteside County. We occasionally receive calls from neighbors, boarding facilities, and other third parties requesting care for client’s pets. The section below lists several examples of groups or individuals who may attempt to seek veterinary care for your pet on your behalf. You will be responsible for the decisions that are made on your behalf, including missed appointments and charges, so please think carefully as you answer. Clinton Veterinary Clinic will refer to this list when someone other than you contacts us about your pets’ care. Please be as general or specific as you deem necessary. We will interpret your answers to the best of our ability. If we refer your pet to a specialist, we will forward all medical notes to assist with your pet’s care.

You are 18 years of age or older. - You have read, understand, and agree to abide our policies and procedures. - You agree to have your pet examined and treated by the doctors at Clinton Veterinary Clinic. - You understand that payment in full is expected at the end of your appointment, or prior to discharge of your pet(s). If you leave the clinic without paying your entire balance, you will be assessed a billing fee of $5.00. Any unpaid balance will accrue at the lesser of 0.79% monthly (9.9% annually) or the maximum amount permitted by law. Any unpaid balance is subject to collection at any time. In the event the account is placed for collection, Clinton Veterinary Clinic shall be entitled to recover all costs of collection, including, but not limited to, court costs and reasonable attorney fees.
